What to expect on your first visit:
When you come in for your first visit, please feel free to park in any of the unmarked spaces in the parking lot, which you can enter on 441 N. Central Ave in Campbell. Our suite (#6) can be found in the far right corner of the complex if you are facing it from the lot. Please come in and have a seat in the waiting room. When seated on the black couch, you will see a chalkboard with your therapist name listed with an office number, just click the number on the light switch on the wall directly under the chalkboard. When you turn the light on this will let your therapist know that you have arrived. Your therapist will walk out to the waiting room to bring you to their office for your appointment.
Who should attend the first session for minor clients?:During the appointment itself, your therapist typically will talk with both the teen and the parent(s) for the first twenty minutes, and then just with the teen for the remainder of the session. If at all possible, we prefer that both parents attend the first session. We value everyone's opinion and unique perspective on the problem!
What forms should I complete for the first session?:In order to save time, please complete the following forms in advance:
I) Patient Information 2) Service Agreement 3) Release of information for your primary care physician. 4) Payment Authorization (keep on file for future appointments) 5)HIPAA Compliance Form 6)Consent for Telehealth Services Please complete all documents BEFORE your scheduled therapy session. Please submit directly online. You can go to the forms tab and click on forms to download. You can either print the forms or scan or fax them to 408-637-5725. You can also bring the hard copies in on your first visit. Insurance:CTFT does not contract directly with any insurance companies. However if you have a PPO with out of network coverage, we are happy to provide you with a "Superbill" that you can use to submit for reimbursement. If you don't have time and don't want the hassle of submitting the claim on your own, we can help. We can submit the claim directly to your insurance company on your behalf. The amount your insurance company reimburses will vary according to the plan you have.

How can I schedule appointments?:At CTFT we offer on line scheduling. For new clients, simply click on the home page and then on the button that says schedule an appointment. For returning clients, you can schedule a new appointment from the website. However in order to cancel, or reschedule you need to create a client portal account. Once you schedule your first appointment, we can send you an email that will allow you to create a portal account. For your convenience we offer text and/or email reminders of your scheduled appointments.

The first session is longer so that we can do a full assessment. The second session is to create a treatment plan with goals together. The number of on going sessions will be determined once we decide the type of therapy that is best for your specific family or teen.
